- Artist or group name and contact details
- Link(s) to audio (streaming link or hi-res file)
- Cover artwork and credits
- Brief biography and summary of achievements (250–500 words)
- Selected category for entry
- Social media handles (Instagram, Twitter/X, TikTok)
- High-resolution press photos (minimum 1500x1500px)
- Music video links (if applying for Best Music Video)
- Release date verification (music released within eligibility period)
- Genre/sub-genre classification
